0012286: Audit Trail does not work on c_invoicetax based based , instead an exception is shown

DescriptionThis exception is shown when opening the audit trail popup on the invoice tax tab

Fixed 12286, Fixed 12265, fixed to audit trail popup
- don't assume ad_element.columnname is unique
- hide go back link when navigated into a child tab
- format modification time without milliseconds
- small ui fixes
